An exciting opportunity has arisen to join the South West London Medicines Optimisation (MO) team in Croydon. The Medicines Optimisation team works in collaboration with other stakeholders in a multidisciplinary way to maximise the health of individual patients through safe, cost effective medicines optimisation of high quality and support integrated care with the community at large.

The purpose of this post is to make a significant contribution to addressing overall health need, integration of care, improvement in quality of care, reduction of health inequalities and support the reduction of unnecessary medications related admissions by maximising high quality, safe and cost-effective prescribing in GP practices within allocated resources.

The Senior Primary Care Pharmacist has a vital role in working collaboratively with general practices to achieve high quality and best value care for the local population. This will involve assisting practices to achieve national standards and meet local targets that have been prioritised by the clinical networks.
