Overview

An exciting opportunity has arisen to work within our Hospital Discharge services as a Trusted Assessor for Local Care Providers.

Are you have significant experience of working as part of multi-disciplinary and multi-organisational team working in social care or health care delivery? Substantial experience of working with older peoples’ services?

You will be at the start of our journey in Buckinghamshire where over the next 12 months we are looking to develop a team of professionals using Trusted Assessment to ensure people are able to be discharged from hospital safely.

The purpose of this role is to support care providers, discharge teams, patients and their families by carrying out assessments on behalf of Care providers. It is aimed that this will free up resources and facilitate discharges from hospital more quickly and smoothly, reducing post-admission problems and readmissions to hospital.

This role offers the opportunity to support the development of this approach as changing and developing processes and evaluating interventions so that the service can develop further will be a key part of the job.

Salary: £33,706 - £40,588 per annum pro rata (NHS AfC: Band 6)

Hours: 37.5 hours per week (Weekends and BH included)

Site: Stoke Mandeville Hospital/across multiple sites

Contract: Fixed term for 6 months
