Job overview

This is an exciting opportunity for an energetic and enthusiastic individual to work with people who have a dementia diagnosis or a functional mental health illness. The Healthcare Assistant (HCA) role is part of the Dementia & Older People Mental Health (DOPMH) team. The successful applicant will work alongside the MDT team, which comprises of the Consultant Psychiatrist, CPN's, Memory Assessment Nurse, Occupational Therapist, Dementia Liaison Nurse, Psychologist and Primary Care Dementia team.

Supporting patients and their carers in the community, either in their home or in a care placement setting. Delivering time specific care planned contact, alongside the patients care co-ordinator.

This is a full-time substantive post.

Main duties of the job

The post holder will be part of a multi-disciplinary team, contributing to the effective provision of Mental Health Services to an identified caseload of service users with a diagnosis of dementia, or over age 75, with functional mental health diagnosis.

The post holder will provide a range of services including practical and emotional support to individual service users as indicated by the individual care plans provided by their care co-ordinators.

Essential criteria
  • NVQ3 in healthcare or equivalent qualification or experience.
  • IT literacy
  • A basic understanding of current National Guidelines and Mental Health Act/ Mental Health Capacity Act requirements.
  • A basic understanding of CPA Process.
