Job summary
Newcastle South PCN are continuing our journey to develop and deliver innovative services to meet the needs of our local population. As part of this, an exciting opportunity has arisen for a Care Co-ordinator specialising in Phlebotomy through the Additional Roles Reimbursement Scheme (ARRS), to join our expanding team.
Main duties of the job
The role will be to primarily co-ordinate and deliver phlebotomy clinics across 6 Practices on behalf of Newcastle South PCN. This will involve setting up clinic schedules, locations, accessing required equipment and consumables and delivering the phlebotomy service at the identified clinics. The post holder will be expected to::
- Work closely with practice teams to co-ordinate the delivery of phlebotomy services across the PCN
- Where appropriate to co-ordinate access to additional services, team members and care packages
- Deliver phlebotomy services within GP Practices across the network
About us
The role will work closely with teams to co-ordinate and deliver phlebotomy clinics across all 6 of our practices. As an experienced phlebotomist you will be expected to provide direct patient care, alongside linking in with multi-disciplinary teams from across the PCN to ensure that patients are getting access to the care and support that they need. This is a new and exciting role for you to develop and strive to support the needs of the PCN going forward.
You will also be required to undertake assessments under the Facilitation of Admission Avoidance Scheme. You would be required to undertake face to face assessments with a specific cohort of patients who are eligible for the scheme.

Job description
Key Service Responsibilities:
- Contribute to the long term condition reviews of patients within primary care as part of the multi-disciplinary team within the practice
- Co-ordinate the clinic schedule, room availability and patient lists for all phlebotomy clinics
- Provide phlebotomy care at each of the organised clinics (see phlebotomy section for more details)
- Undertake Assessments under the Facilitation of Admission Avoidance Scheme
- Contribute to the delivery of long term condition clinics as and when required, through providing basic patient care such as phlebotomy, blood pressure and measurements
- Work alongside our team of Care Co-ordinators to ensure that all patients are effectively accessing care in accordance with their needs and wishes
- Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care
- Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level
- Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals
- Support the coordination and delivery of MDTs across the PCN
Phlebotomy Responsibilities:
- Obtaining patient consent for the collection of a blood sample(s)
- Performing venepuncture in accordance with practice protocols
- Maintaining accurate patient records
- The correct labelling, packaging and storing of ( various) samples
- Ensuring samples are sent to the laboratory in a timely manner
- Maintaining an effective liaison with the laboratory staff
- Support and participate in shared learning within the practice
Facilitation of Facilitation of Admission Avoidance Scheme:
- Identify eligible patients from a specified cohort
- Ensure clinics are fully booked with identified patients
- Carry out face to face assessments
- Ensure case management and care plans are written up and recorded at the end of each appointment, ensuring any referrals or actions are facilitated
Administrative Responsibilities:
- Work with the GPs and other primary care professionals within the PCN to identify patients requiring access to phlebotomy
- Raise awareness within the PCN of the service and how to access it
- To work as a key member of the MDT to help support the development of effective MDT meetings.
- To ensure that action points identified within the MDT are recorded and followed up
Personal/Professional development:
- Participate in any training programme implemented by the PCN as part of this employment, such training to include:
- Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development
- Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work
- To work in accordance with health and safety policies and procedures including reporting and recording any health and safety incident or accident
- Adhere to host employer’s adult and children safeguarding policies and procedures

Qualifications / Knowledge

- NVQ Level 3 or equivalent level of knowledge in office procedures
Experience

- Experience of venepuncture

- Experience in a patient focused environment
- Evidence of experience in wide range of administrative systems and software programmes

- Experience of supporting service improvement
- Previous experience in NHS / Primary Care / Local Authority role
